Your First Appointment

Please complete and submit the appropriate forms from the forms link 48-72 hours prior to your scheduled appointment.
We require all new patients to arrive 30 minutes prior to your scheduled appointment to allow adequate time for registration. Late arrivals will be rescheduled to the next available appointment.
All established patients must arrive no later than 15 minutes prior to your scheduled appointment time.

Please bring the following:

  • Insurance Cards for all current policies
  • Photo ID
  • List of current medications (prescription and over-the-counter) including dosage and frequency
  • Recent test results (labs and imaging) and all other relevant medical records

MUST COMPLETE NEW PATIENT PAPERWORK AND ARRIVE 30 MINUTES PRIOR TO APPOINTMENT

CWH&W provides insurance billing. You are responsible for co-payments, co-insurance, and deductibles at time of service, as well as any charges that are not covered by your insurance company.
